PERFECT IMAGE CLIENT INTEGRATION / OPEN ACCOUNT APPLICATION FORM

Online applicants agree to the following terms and conditions of the Perfect Image Client Integration / Open Account Application Policy:

1. The applicant represents that he or she has authority to enter into this agreement with Perfect Image, and agrees to the following terms and conditions.
2. The applicant is assured that all work performed by Perfect Image will be produced in accordance with industry standard Best Practices for the Print Industry, which are available on the Perfect Image Helpful Resources and Forms page of this Web site.
3. The applicant gives permission to the bank and business references provided to disclose their experience with the applicant to Perfect Image, and acknowledges that the information is to be used in consideration of granting an account to the applicant.
4. Terms are collect on delivery (COD) unless otherwise stated in writing.
5. For any balances due to Perfect Image after a period of 30 days, the applicant will pay interest at the rate of one and one half percent (1.5%) per month.
6. Title for work and services performed by Perfect Image shall not pass to the applicant until all money due to Perfect Image is paid in full.
7. In the event the applicant defaults on these terms and conditions, all legal fees, court costs and collection fees are to be paid by the applicant.

CREDIT CARD BILLING AUTHORIZATION FORM

1. To protect client security, when Perfect Image's online Credit Card Billing Authorization Form is used to authorize payment for a print project, Perfect Image does not retain credit card information after payment is processed.
2. Only a duly authorized representative of the company named in the Credit Card Billing Authorization Form may authorize payment to Perfect Image using this Form.
3. All Credit Card Billing Authorization Forms submitted to Perfect Image via this Web site are one-time authorizations to process payment for services to be provided by Perfect Image, which are described in the Form.
4. Should a charge be denied, the representative signing the Credit Card Billing Authorization Form acknowledges that Perfect Image must be provided an alternative form of payment, which can include a check, money order and/or cash, before Perfect Image will proceed with or deliver the project, depending on the status of the job.
